What we do

The Vale of Glamorgan Long Term Care Service supports people who cannot meet their wellbeing outcomes without care and support. The service consists of:

Social workers who undertake assessments, Social Care Officers who support in reviewing long standing care provision. The team works with people and their families to support them to meet their own wellbeing outcomes. This could involve providing or facilitating social work support, liaising with partner agencies, providing domiciliary care, day services, respite as well as residential and nursing care.

The Longer Care Team works with people living in thier own homes, supported accommodation and care homes to ensure that people are getting the right care to meet their needs and avoid unnecessary hospital admissions.

Who do we support

Adults who are 18 years or over and live in the Vale of Glamorgan
-Those who look after adults who are 18 and over (carers)

It is more likely you will be assessed as needing care and support if you:
• are older or frail
• have physical disability
• are an older adult with mental health issues
• not able to protect self from harm

Or you are a carer supporting an adult or adults with care and support needs.

Is there a charge to use this service?

Depends - You will not be charged for social work support but you may be asked to pay toward the cost of any Care and Support that you receive dependent upon a Financial Assessment. You will be informed of all charges throughout the process.

Can anyone use this service?

Referrals can be made by individuals, family and friends, as well as other professionals by contacting C1V.
